Website Privacy Policy
Review draft — prepared October 5, 2026. Proposed effective date: October 5, 2026, subject to practice approval before publication.
Scope
This draft concerns information associated with the Crowned Psychiatry website. It is separate from the practice’s clinical Notice of Privacy Practices. Approved clinical documentation must be supplied and reviewed before a Notice of Privacy Practices is posted.
Information you choose to provide
The general inquiry form includes name, email, subject, and a general inquiry. The newsletter sign-up includes email and optional name. We also provide a practice email address and phone number for general questions. Please do not submit diagnoses, medication details, symptoms, or other sensitive health information through ordinary website forms or email. These channels are not monitored for emergencies.
Forms and newsletter setup
The newsletter block is connected to Squarespace storage. No Google Drive, Zapier, or MailChimp connection was shown in its storage settings during this review. The general inquiry form still requires its delivery/storage configuration to be confirmed before launch. Form routing, who has access, retention, response procedures, newsletter consent, and unsubscribe handling must be verified before this draft becomes an effective policy. The practice uses inquiry information to address general questions and newsletter details for requested practice news, subject to the approved setup.
Hosting, cookies, and analytics
This website is hosted on Squarespace. Squarespace describes collecting technical information needed to operate its websites and supporting site analytics, including information about browsers, devices, pages viewed, referring links, and network information. Cookies are small files stored on your device. The exact cookie banner, analytics settings, and any additional tracking used by Crowned Psychiatry must be confirmed before publication; this draft does not assert that optional cookies are blocked or that particular third-party trackers are absent.
You can manage cookies in your browser. Blocking cookies may affect website features. Learn more through Squarespace’s Privacy Policy and its cookie documentation.
External booking and other links
Appointment buttons open our existing OptiMantra booking destination. Information entered there is handled through that service and the practice’s approved clinical processes, rather than the ordinary website inquiry form. Review the policies shown by OptiMantra before entering information. Instagram, Facebook, and linked educational sites are operated by others and have their own privacy practices.
Sharing, retention, and requests
Squarespace and any verified service providers may process information needed to operate the website and its configured features. Practice-specific sharing, retention periods, deletion procedures, marketing uses, applicable privacy rights, and any legal disclosures require confirmation. This draft makes no claim of HIPAA compliance, guaranteed security, a particular retention period, or verified sale/sharing practices.
To ask about website information or request assistance, email info@crownedpsychiatry.com or call 951-292-9189. Do not include sensitive clinical details in an ordinary email. Request handling will be subject to applicable law and verified practice procedures.
Children and family care
The practice offers care for children and adolescents, but this website provides general educational and practice information. Parents or guardians should contact the practice for appropriate intake instructions. Do not submit a child’s sensitive health information through ordinary website forms.
Updates and contact
The approved policy will identify its effective date and be updated when website data practices change. Crowned Psychiatry’s mailing address is 14910 Perris Blvd, Ste B PMB 1015, Moreno Valley, CA 92553. Care is provided online.
Before publication: Confirm all form/storage destinations, newsletter procedures, cookie and analytics settings, connected services, access controls, retention and sharing practices, applicable rights, and the approved clinical Notice of Privacy Practices. Practice and legal review are required; this draft does not claim legal compliance.
